These are set forth in the five components that make up CHAMPIONS:
Community-Based Cardiovascular Risk Screening
Screening individuals for hypertension, obesity, diabetes mellitus, hyperlipidemia, and peripheral artery disease, with a focus on populations that do not access specialty care.
Health Education
Educating populations on cardiovascular risk factors, self-care, and available health services. This effort is conducted in collaboration with community-based nonprofits and health service providers.
Medical Student Training
Training medical students through fellowships focused on course and curriculum development, along with training for other medical professionals and pre-med volunteers in public outreach and care for underrepresented communities. These efforts emphasize partnerships with academic and community health providers. Key targets include medical schools, public health programs, and local clinics.
Capacity Building for Primary Care Providers
Enhancing the skills of community clinic staff and primary care providers to care for high-risk individuals identified during screenings. This includes creating a network that connects individuals with insurance and healthcare services, while integrating prevention and risk management efforts within community-based clinics. Targeted partners include primary care providers, community clinics, and insurance/payers.
Research
Investigating barriers and motivators to prevention-oriented health behavior, with the goal of improving prevention strategies, increasing health engagement, and enhancing the effectiveness of educational initiatives on health outcomes.
